The SmartCARE project designed an ICT platform of advanced e-health services, which allow to physicians and patients to meet and manage daily activities in a collaborative way, enabling an optimization of healthcare rehabilitation at home within a therapeutic process. From the technical side, the system is based on the use of sensors for movement tracking and measurement of all the parameters used for reconstructing the patient’s biofeedback picture during the neuro-motor and neuro-cognitive rehabilitation exercises. The SmartCARE platform enables services assuring a constant verification of the therapeutic process without requiring the presence on-site of the medical staff. In a scenario in which the continuous aging of European population increases the demand for healthcare services and related costs, the SmartCARE ICT-based healthcare tools can help in reducing the health operational costs without reducing the clinical effectiveness of care and assistance. SmartCARE becomes useful to prolong the period of rehabilitation after the hospitalization, to guarantee a punctual assessment of the efficacy of the treatment via multimodal measurements, to facilitate the communication between medical staff and caregivers, and to allow a safety control of patients from both medical staff and family components. Moreover, satellite communication allows all citizens to access the SmartCARE services eliminating digital barriers. SmartCARE has two main subsystems cooperating in order to offer rehabilitation, monitoring and communication services at home: i) Health Station: offers the services of rehabilitation and communication to the final users (patients, caregivers) at their home; and ii) inCloud Service: supports the services offered by the Subsystem at home in terms of coordination of the therapies, management of centralized information, control of health status of patients, privacy and security. SmartCARE has been funded by the European Space Agency (E.S.A.) within the Satcom Application in the Health thematic area.
