SHaDE: Smart Holistic Diabetes Ecosystem

The project was presented in response to a Pre-Commercial Procurement Call for Tender issued as part of the European ProEmpower project funded by the European Commission under the Horizon 2020 program and coordinated by the Ministry of Health of the Republic of Turkey. The first phase of the project that will last 2 months has now been approved.

The project has the aim at designing an ecosystem based on specific building blocks for diabetes management, such as early detection, patient-professional co-ordination, personal decision support, comprehensive diabetes training offer. The ecosystem has the objective to support the patients in self-monitoring on different dimensions, like: nutrition, physical activities, habits, psychological and physiological aspects according to a multi-disciplinary care model, and it also influences the nutrition and exercise habits in a permanent and durable way thanks to personalized coaching, on the base of patients’ proper lifestyle. Moreover, the above mentioned self-monitoring is performed with different approaches (manual, automatic and semiautomatic data collection, also through the integration of supported devices) to increase the measures that the ecosystem can track, optimising, among the others, glycaemic control for a good diabetes management.
