The project was presented as part of the MISE Call Horizon 2020 – PON I&C 2014-20 by a partnership comprising 1 University and 2 SMEs. ICAR-CNR provides its consultancy to an SME on the industrial research activities of the project.

The goal of Base Line is to define a highly performing and innovative architectural platform designed to support health professionals involved in the treatment of patients with chronic diseases, with particular reference to Chronic Kidney Disease (CKD). The platform will present features of modularity and scalability and will enable healthcare professionals to improve the quality of patient care through the use of innovative tools and techniques implemented and made available. In recent years, in fact, the need for clinicians in the clinical-health sector is growing more and more to be supported for the management of a huge amount of data that is not always homogeneous. The enabling platforms are able to integrate heterogeneous information sources of social-health data in order to provide effective tools for monitoring, diagnosis and treatment of patients with chronic diseases and to facilitate the analysis of large amounts of data (Big Data Analytics) , through which it is possible to establish a knowledge base for the definition of new therapeutic pathways. The data processed, analyzed and archived by the system, are sensitive and for this reason particular attention will be paid to the aspects related to data protection and privacy management. The intrinsic and fundamental value of an integrated platform is to be able to condense and integrate the different skills that participate in the management of the path of treatment of chronic diseases (specialized medical personnel, family doctors, nurses, care givers, dieticians, professional trainers, etc.) through suitably designed workflow services in order to obtain an overview of the various factors, not just clinical, that can affect the evolution of a disease. The Base Line platform is mainly intended for use in specialized clinical departments for the treatment of KCD. It is configured, in fact, as a tool to support the activity of different professionals involved in the patient care process, such as nephrologists, endocrinologists, psychologists, nutritionists and nurses, as it offers a set of features for the integrated management of affected patients from KCD. The test and validation of the platform will be performed on the KCD, but the added and innovative value of the Base Line platform is to be easily adaptable and customizable for other classes of existing chronic diseases.
